A sexually oriented business licensed under this chapter shall file a monthly report with the department under rules adopted by the department. The report must include: (A) the name, address, and date of birth of all performers appearing during the month on the business premises; and (B) the name, address, and date of birth of all persons acting as managers or assistant managers, and employees.
