Any user discharging one hundred kilograms or more of dangerous waste in any calendar month to the POTW where the pollutants are not reported through self-monitoring under an applicable state waste discharge permit shall report to the director and the Department the following information to the extent that it is known or readily available to the user:

A. The name of the dangerous waste as set forth in Chapter 173-303 WAC, and the dangerous waste number;

B. The specific hazardous constituents;

C. The estimated mass and concentration of such constituents in waste streams discharged during the calendar month;

D. The type of discharge (continuous, batch, or other); and

E. The estimated mass of dangerous waste constituents in waste streams expected to be discharged in the next twelve months. (Ord. 016/2021 § 2; Ord. 011/2004)
