To obtain a permit, the applicant shall file an application in writing on a form provided by the city for that purpose, including any attachments as may be required by the city, and in accordance with instructions provided in the application form. This application shall be filed not less than ten days in advance of the date right-of-way disturbance is proposed to begin. (Ord. 1014, 1993)
